States push ahead with Medicaid work rules

A few are starting early, including a Nebraska program launching Friday.

A handful of Republican-led states are planning to implement new work requirements under the One Big Beautiful Bill Act months ahead of the 2027 deadline and often with more restrictions than the federal law requires, according to data released Thursday. 

The annual survey of 43 state Medicaid and CHIP programs, conducted by the health policy and research organization KFF and the Georgetown University Center for Children and Families, offers the most comprehensive look to date at how states are approaching new requirements under the 2025 federal law — the most most significant structural overhaul of the federal Medicaid law in decades.
